- Hollywood Mega Smile 1500- The above benefits with this plan, covered benefits are paid at the 80th percentile of usual charges.
- Hollywood Mega Smile 2000- The Hollywood Mega Smile 2000 Plan is an indemnity plan. This means you may go to any licensed dentist that you wish.
- Hollywood Smile Premier 1500- This is a PPO plan.
- Hollywood Smile Premier 2500- This is a PPO plan. Money-saving Preventive Plus benefits are offered with this plan. Type 1 Preventive procedures are not deducted from the plan's annual maximum benefit. This will save all the annual benefits to help pay for more expensive procedures.
